What you need
Have the last account or the inventory, and the year’s bank and brokerage statements. Lines A-1 and A-7 take their values from whichever of the two is more recent.
Every line above zero needs an itemized list on a separate sheet. Head each one with its section, schedule and type, such as “Section A, Schedule A-2, Gain on personal estate”. Enter 0 on empty lines instead of leaving them blank.
The example follows Silas Whitcomb, an invented ward whose guardian was appointed 03/10/2024. This second account covers 03/10/2025 to 03/10/2026.
Subtract line 2 from line 1
The worksheet adds up what the guardian started with and took in (Section A), and what went out (Section B). The summary on page 3 subtracts one from the other. The result is the balance you manage, real estate included.

The sale
Silas’s last account carried 100 fund shares at $52.00 each. His guardian sold 50 for $3,140.00. Those shares were already in A-1 at $2,600.00, so A-2 takes only the $540.00 gain. A sale below the carried value goes on B-1 as a loss.
The court rule asks the ending inventory to itemize historical cost and current value separately, when current value is known. The 50 shares left are carried at $52.00 and trade at $66.80, so at current value line 3 would read $191,284.50. The rule lets you put that $740.00 rise on A-2 as an unrealized gain. The motion to allow
Page 4 asks the court to allow the account and certifies that the itemized lists are attached. A guardian files the motion every year unless the court finds good cause not to. V.R.P.P. 66(f) (opens in a new tab)
Serve copies on everyone entitled to notice. Unless the court directs otherwise, anyone who objects must do so in writing at least 7 days before the hearing. With no objection, the court may allow the account without a hearing. V.R.P.P. 66(c), (g) (opens in a new tab)
The Judiciary’s fee schedule (opens in a new tab) lists no fee for a guardianship account.
The final account
There is no separate form. Tick “This is a final account” on 700-00089PAG when the guardianship ends. §3076(c) (opens in a new tab) No rule sets a number of days for filing it.
If the ward dies, you keep the power to make and file the account for up to two years, or until an executor or administrator is appointed. 14 V.S.A. §3077(e) (opens in a new tab)
Once you have paid out what the final account shows, file the Closing Report and Discharge (700-00152PAG (opens in a new tab)) within a year of the order allowing it, or later if the court allows. Attach a Receipt (700-00153) from each person paid. The judge’s discharge order is on the same page.
Common mistakes
The whole sale price on A-2 or A-3
A-1 already holds what was sold. Only the gain goes on A-2, and only a loss goes on B-1.
Leaving the house out of line 3
Real estate stays in line 3. It is carried at its inventory or last-account value, plus any gain on A-8 or less any loss on B-7. Line 3 is not your bank balance.
A fee the court didn’t allow
B-5 takes only guardian fees the court allowed on your motion, with papers to support it. Without that order, none can be paid until the annual report is filed. §3076(d) (opens in a new tab)

Social Security only
If you manage only the ward’s Social Security, this is the wrong form. Attach the one-page 700-00400 (opens in a new tab), Accounting When Only Income is Social Security Benefits, to the annual report instead.
It asks the payee’s questions: benefits received over twelve months, spent on food and housing, spent on other things, and saved. Attach a current statement for any savings.
