What it asks for
Tick Guardianship as the case type. List every item with its fair market value on the date you were appointed, and any mortgage or lien, then total the values.

You need not hire an appraiser unless the court orders it. V.R.P.P. 66(h) (opens in a new tab) If you use one, list the appraiser’s name and address for each item appraised. The form has a block for it.
Common mistakes
Only what the ward owns alone
A guardian also lists jointly owned property and every beneficial interest, such as a life estate or a share of a trust. Give its value and the names and addresses of the other owners. The court rule asks for this, and the form doesn’t mention it. V.R.P.P. 66(b)(2) (opens in a new tab)
Missing attachments
Real estate needs a copy of the deed, and the latest property tax bill if it wasn’t appraised. The form says “or”, but the court rule asks for both. A mobile home needs its bill of sale, and its tax bill if not appraised. A vehicle needs its title or registration. Tick each one attached on page 1. V.R.P.P. 66(b)(1) (opens in a new tab)
A bank account with no address
The form asks for the bank’s name. The rule also asks for its address, with the last four digits of the account number, never the whole number. V.R.P.P. 66(b)(1)(E) (opens in a new tab)
Values on the wrong date
The form’s instructions value property at a decedent’s date of death. For a guardianship, use the day you were appointed.
Your first account
Lines A-1 and A-7 of the first 700-00089PAG carry the personal property and real estate from this inventory. Property found later goes on A-4 or A-9, as not listed on the inventory.
