What you need
Have the inventory or last year’s account, the year’s statements, and proof of every payment. Line 1 is the personal property on the inventory, or held at the end of the last account.
The example follows Evelyn Toomes, an invented ward whose guardian qualified 03/10/2025 and chose a fiscal year ending 02/28. This first account covers 03/10/2025 to 02/28/2026 and was due 03/30/2026.
Part I must equal Part II
Part I works from the opening balance to the balance at the end. Part II lists where that balance is held. The form says its total must equal Part I line 9.

Gains and losses
Evelyn’s inventory listed 50 fund shares at $60.00 each. Her guardian sold 20 for $1,050.00, $150.00 less than their inventory value, so the loss goes on line 2. The $1,050.00 is not a receipt. It was already counted in line 1.
A sale above inventory value works the other way. Only the gain is a receipt, in Part III. A change in value on something not sold is not a receipt. It stays at its inventory value.
Filing it
Bring the bank statements and investment statements that show the balance. The clerk examines them and certifies on the account that they match. G.S. 35A-1268 (opens in a new tab)
Court costs are charged on Total Receipts: 40 cents for each $100, and at least $15. G.S. 7A-307 (opens in a new tab)
With your first account, the clerk approves your fiscal year on AOC-E-514 and writes in the date the next account is due. A year that ends outside the 11 to 12 months is rejected. G.S. 35A-1264 (opens in a new tab)
The final account
The guardianship ends when the ward dies, turns 18, is restored to competency, or moves to another state by final order. File AOC-E-506 with Final checked within 60 days. G.S. 35A-1266 (opens in a new tab)
It runs from the end of the last annual account, and line 9 is zero. After a death, Part V shows the estate delivered to the personal representative. When the clerk approves it, the clerk enters an order discharging you.
Common mistakes
Letting the date pass
A late guardian is ordered to account within 20 days of service. After that come contempt and removal, and you pay the costs yourself. G.S. 35A-1265 (opens in a new tab)
A payment with no proof

Account numbers on the form
Part II asks for each bank, not the account numbers. Leave them off.
