Guardian of the person only? You don’t file this. The account you file each year after is CC-GN-012.

What you need

Values are as of the date you were appointed. Attach a statement or valuation for each item from that date, or the most recent one.

The rule sets one deadline, 60 days after appointment. The form’s own note says to file it “each year”. Ask the trust clerk which your court expects.

Common mistakes

A full account number

Accounts, policies and cards are listed by the last four digits only.

A section left blank

If a section doesn’t apply, the form asks you to write “Not applicable” or “N/A”.

A mortgage listed twice

A mortgage or lien shown with the real estate or vehicle it secures does not go in Part II again. Part II is for the other debts.

Every household item itemized

List each item worth over $2,500. Describe the rest together, such as “furniture and clothing”.

After you file

The court doesn’t examine the assets themselves unless it directs otherwise. Any interested person who asks for a copy of the inventory gets one, from you or the trust clerk. Rule 10-707(b)–(c) (opens in a new tab)

Your first account

The first CC-GN-012 starts from these figures. Its summary asks for the value on the last account, “or Inventory if this is the first account”.
