What it asks for
Every figure is as of the date of the order appointing you, not the day you file.

The statute also asks how each item is titled, and for every beneficiary designation, including pay-on-death and transfer-on-death. The form has no box for them. 59-30,181(a) (opens in a new tab)
The last page is a certificate of service. It lists the ward, her attorney and the guardian ad litem, each served by mail or electronically.

Common mistakes
Yearly income in Part 2
Part 2 asks for income per month. A $1,486.00 monthly benefit goes in as $1,486.00.
A shared vehicle at full value
Subtract the loan to get net value, then enter the ward’s percentage and the value of her share.
Belongings not itemized
Attach a list of every item worth $1,000 or more, including safe deposit box contents.
Your first account
The first Conservator’s Report and Annual Accounting starts on your appointment date. Its Worksheet A starts from this inventory. Its line 1, the beginning net asset value, is what the ward owned less what she owed on your appointment date.
