What you need
Have the last report or the inventory, the year’s statements, and every receipt. The form asks you to keep the receipts in case the judge asks for them.
The example follows Myrtle Haskins, an invented ward whose conservator was appointed 06/10/2024. Her first report ended 06/30/2025. This second report covers 06/30/2025 to 06/30/2026. Her order made it due 07/30/2026.
Line 6 is next year’s line 1
Line 1 is the Beginning Net Asset Value from Worksheet A: everything the ward owns, less what she owes. The form says it is also the ending balance from the last accounting or inventory. So line 6 should match Worksheet A’s Ending Net Asset Value.

Myrtle owes nothing, so neither side of Worksheet A has liabilities.
The revaluation and the sale
The county’s new appraisal put the home at $148,500.00, up $6,500.00. That goes on line 4, with an itemized list attached.
Myrtle’s car was carried at $9,500.00. With the court’s approval, her conservator sold it for $8,200.00. Only the $1,300.00 loss goes on line 5, with its own list. The $8,200.00 landed in checking, where it replaced the car on Worksheet A. K.S.A. 59-30,179(a)(2) (opens in a new tab)
The dates come from your order
The first report starts on the day you were appointed. Each later one starts on the last one’s ending date.
Three sources describe the end of the year. Rule 109 uses the 12 months before the anniversary of the case filing, due 30 days later, unless the court orders otherwise. Sup. Ct. R. 109(a) (opens in a new tab)

The form says the period ends on the last day of the month of your appointment’s anniversary. Your own order controls, so read it first.
Common mistakes
Paying your own fee before it is approved
A conservator’s fee needs the court’s approval before it is paid. The form has a fee request with a log of your hours. K.S.A. 59-30,119(c) (opens in a new tab)
Bank statements in place of the worksheets
The form says attaching statements does not replace completing it. Fill in Worksheets A to C. The statute also asks for the latest bank, investment and debt statements where you can get them, with account numbers and Social Security number redacted. 59-30,182(b)(5) (opens in a new tab)
A missing bond renewal

After you file
The court approves the report after notice and a hearing. Approval settles your liability for what the report adequately disclosed. 59-30,182(h)–(i) (opens in a new tab)
The final report is this form with Final checked, filed with a petition for discharge. Its approval discharges you. 59-30,182(j) (opens in a new tab)
