Iowa Form 7, the Conservator’s Annual Report

Iowa’s yearly report for a conservator. It also asks the court to approve next year’s budget and powers.

The 60 days are the form’s. The statute set the same until 2024 and now gives no day count. §633.670(3) (opens in a new tab)

What you need

Have last year’s report or the inventory, the year’s bank and brokerage statements, and your receipts. They must be available to the court on demand. You also file a new Form 6 as of the period’s end.

The Code does not say when a conservator’s reporting year ends. The report covers everything since the last one closed. §633.671 (opens in a new tab)

The example follows Delbert Vanderhoef, an invented protected person. His report covers 06/01/2025 to 05/31/2026, so under the form it was due by 07/30/2026.

5.A plus 5.B, less 5.C and 5.D, equals 5.E

Section 5 counts the funds on hand. The form does not print this sum, but its figures only agree if it holds. The totals are for the whole period. Where a 5.D line is given monthly, tick monthly and multiply it out for the total. The example enters every line as its period figure and ticks annual in 5.D.

Section 4

Section 4 is every asset, from Form 6. Delbert also holds household goods worth $800.00 and 120 fund shares, which rose from $80.00 to $84.50. So 4.A is $24,686.40 and 4.B is $27,916.33. The rise of $3,229.93 is the $2,689.93 added to funds plus the $540.00 gain on the shares. 4.B should agree with the new Form 6.

Next year’s budget and powers

Section 7 proposes a budget for the next period. Section 11 asks the court to approve it and your general powers, including a monthly cap on miscellaneous spending.

The order approving the report sets your powers until the next order, and new letters issue. §633.642(3)–(4) (opens in a new tab) No order approves the report until the docketed court costs are paid or provided for. A significant change to your plan needs an amended plan on Form 5, not this report. Other powers need a request on Form 3.

Copies

Serve a copy on the protected person, their attorney and court visitor, if any, and anyone the court names. The form calls the visitor the court advisor. §633.670(2) (opens in a new tab)

The final report

The final report is Rule 7.12 Form 8. It is due within 30 days after your removal, and with a resignation before the court accepts it. When the conservatorship ends by death, majority or court order, it is due within 60 days. §633.670(4) (opens in a new tab)

It carries the same parts as this report, plus a proposed plan for the assets, with a Form 6 attached. You then account in full to the protected person, or after a death to the personal representative, with notice (Iowa Code §633.677 (opens in a new tab)).

Common mistakes

“Same as last report”

The form rules out answers like that. Give this year’s facts in full, even if unchanged.

One payment in both 5.C and 5.D

Credit card and car loan payments have lines in both parts. Enter each payment once, or 5.E comes out short.

Cash with no explanation

Explain anything paid in cash or by a cash withdrawal. The same goes for debts owed to you or shared with someone else. §633.670(3)(b)(2) (opens in a new tab)
