What it asks for
Most rows ask what the asset would sell for, any debt on it and to whom, and the net value. Life insurance and bank accounts ask the cash value less any loan. Each row also asks whether the asset is jointly owned. The statute names no valuation date for this first inventory.

Common mistakes
Filing it on its own
The inventory is attached to the initial plan, Rule 7.12 Form 5. Both are due within 90 days, and the court reviews them together. §633.670(2) (opens in a new tab)
Life insurance at its death benefit
Part 1.D asks for the cash value. Subtract any loan against it to get the net.
A late find sent in as a new inventory

Skipping the copies
Give copies to the protected person, their attorney and court visitor, if any, and anyone the court names. The form calls the visitor the court advisor. §633.670(2) (opens in a new tab)

Your first account
Each annual report attaches a new Form 6 as of the period’s end. Its section 4 sets that total beside the total at the end of the period before.
