PG-220, the Conservatorship Implementation Report and Inventory

A copy goes to the protected person, if 14 or older and able to understand it, and to any parent or guardian they live with.

The distribution date is on the clerk’s certificate at the foot of your order, not the day it was signed. The order may set a different due date. PG-415 ¶5.a (opens in a new tab)

Sections A to C cover you, the protected person, and how you will handle housing, care and contacts. Section D is the money:

Attach every monthly bank statement from the date of your appointment order to the date of the report. For any other account with no statement in that time, attach its latest one. You swear to the report under penalty of perjury.

After you file

Guardianship cases are still on paper, so file by mail or in person. There is no separate fee: the $150 paid when the case was opened covers every service.

Serve a copy on the protected person, their attorney or guardian ad litem, the family member they live with, and anyone else the form lists. The certificate of service is part of the form. Probate Rule 14(h) (opens in a new tab)

The court reviews the report within 30 days. It sends an approval letter or a deficiency notice, which you answer on PG-360.

Common mistakes

Values as of the appointment

The form asks for assets and debts as of the date of the report. Use the balances on that day.

Yearly amounts in D.1

Income must be monthly. The form says to divide yearly amounts by 12 and quarterly amounts by 3.

Questions left blank

The form says not to leave any question blank. Write “n/a”, or explain what you are still finding out in section E.

Your first account

The form calls this report the baseline the court compares next year’s against. The first PG-225 covers 12 months starting on the 1st of the month your order was signed, unless the order sets other dates, so its period begins before this report. Its income and expenses are annual totals, not monthly.
