What it asks for
You swear the inventory is true and correct, under penalty of perjury.

Attach the list of personal property worth $1,000 or more. Bank statements and other financial papers are filed under seal. GR 22 (opens in a new tab)

Common mistakes
Values from the day you filed
Every balance and value is as of the date of the order appointing you. Use the statement that covers that day.
Full account numbers
The form asks for the last four digits only.
Yearly income in part 2
Part 2 asks for each source per month. A pension of $4,830.00 a year goes in as $402.50.
Your first account
Your first GDN R 204 report usually starts on the day you were appointed, the date this inventory values. Check the period in your order. Its balance sheet asks for market value. Real estate listed here at its tax-assessed value may need a different figure there.
