What you need
Have your last report or the inventory, the year’s bank and brokerage statements, and the court orders behind each payment. The court may ask for receipts and cancelled checks.
The example follows Lorraine Pettigrew, an invented protected person whose conservator was appointed 03/10/2024. This is her second report, covering 03/11/2025 to 03/10/2026, and it was due 03/10/2026.
Line 19a is last report’s line 19e
Add the year’s receipts to the beginning balance and subtract what was paid out. Line 19e carries to next year’s line 19a exactly. On the first report, line 19a comes from the inventory.

What line 19a counts
The form doesn’t say which inventory figure starts the first report. This example holds only cash, so line 19e matches the bank balances on line 18h. She has no cash on hand, so line 18i is the same. Ask your probate court if it wants another figure.
The sale
With the court’s approval, Lorraine’s conservator sold 50 of her 100 fund shares for $4,215.00. The form has no line for a gain or a loss. Because this summary follows cash, the whole $4,215.00 is a receipt. The 50 shares left go on line 20 at their current value, $4,310.00.
Long form or short form
Both forms carry the same summary: lines 19a to 19e on the long form, 5a to 5e on the short. The long form also asks about the bond, lawsuits, bankruptcy, inheritances, life insurance, a safe deposit box, each bank account, investments and debts. Part D has nine statements you initial.
Neither form says who uses which, and the Conservator Contract lists both. Ask the probate court that appointed you which one it wants.

Common mistakes
Paying bills without an order
The statute lets you spend without a further order only in line with a court-approved financial plan. §62-5-423(A) (opens in a new tab) The Conservator Contract you signed says no payments without prior written court approval. Form 543GC, item 8 (opens in a new tab)
New assets left off the inventory
If you find or buy an asset the inventory does not list, the long form tells you to file an amended inventory on 550GC.
Filed without sending copies
Deliver the copies first, then complete the proof of delivery on the form. §62-5-416(C) (opens in a new tab)
After you file
The probate court reviews the report itself. It may appoint a guardian ad litem to review it and may order the assets examined. §62-5-416(D)–(E) (opens in a new tab)
To ask for more time, file Form 586GC (opens in a new tab). A report left unfiled can bring a fine or jail for contempt, as the Conservator Contract warns. Form 543GC, item 17 (opens in a new tab)
The final report
A report is also due on resignation or removal, when the minority or disability ends, on the protected person’s death, and whenever the court directs. §62-5-416(A) (opens in a new tab) File the same form, marked as a final report with an application for discharge. The court’s order sets its due date.
