What it asks for
One page: a free-form list, then a recapitulation. Many counties add their own requirements, so read your court’s local rules too.

Fees and late filing
The court charges $7 to file the inventory, or $10 with an appraisal. Counties add fees of their own. R.C. 2101.16(A) (opens in a new tab)
A late inventory draws a citation to file it or appear. At the hearing the court can deny your fees, charge a $100 penalty and $25 costs, or find you in contempt. R.C. 2109.31 (opens in a new tab)
If the inventory is still missing 30 days after the court’s notice, the court must remove you. R.C. 2111.14(A)(1) (opens in a new tab)
Common mistakes
A monthly figure on Other Annual Income
The line asks for a year. A $1,640.00 monthly pension goes in as $19,680.00.
Spending before the inventory is filed
The court will not approve an application to spend the ward’s funds until the inventory is on file. Sup.R. 8.31(B) (opens in a new tab) Without good cause, it will not order a sale of personal property before then either. Civ.R. 73.14 (opens in a new tab)
An extension you did not sign
The court grants more time only on an application you sign yourself. Civ.R. 73.28(B)(2) (opens in a new tab)
Your first account
The first line of your first Form 15.8 is the balance from the previous account. On a first account, that is this inventory’s personal and real estate values, not its Total line, which adds a year of rent and income.
