Every conservator

Mail a copy to everyone your order names: the protected person, any co-conservator or guardian, and the interested persons listed. §30.1-29-18 (opens in a new tab) Then file the report with the Confidential Information Form, the Contact Information Form and a Declaration of Service by Mail. What it asks for

List everything the protected person owns or has an interest in, so far as you know. Each line takes a description, a value or balance, and where it is. You sign the report under penalty of perjury.

Common mistakes

Every household item listed

Clothing, furniture, appliances and housewares go in as one estimated total. List only high-value items, like jewelry, art or antiques, one by one.

A mortgage netted against the house

Paragraph 4 takes the full value. The loan goes in paragraph 7, and line 10 subtracts it. The total estate value can be negative.

Full account numbers on the report

Write only the last four digits. The full numbers go on the Confidential Information Form, which is filed but not mailed to anyone.

Your first account

The court’s instructions make paragraph 2’s total, cash and checking, line 1 of the first Conservatorship Annual Report. Put only the accounts you pay bills from in paragraph 2.
