What it asks for
Seven schedules, totalled on page 1. List every asset, including those outside New Jersey. Bank, brokerage, annuity and life insurance values are as of the day you were appointed.

| E | Other personal property: vehicles, collections, business interests |
| F | Debts, including a mortgage or car loan, subtracted from the gross value |
| G | Every source of income, at its yearly amount |

Common mistakes
An empty schedule left blank
Every schedule gets an answer. Write “NONE” where the ward has nothing.
Face value added into the total
Schedules A and B have two value columns. Only market value goes into the total.
Monthly income on page 1
Page 1 labels Schedule G “Monthly Income”, but the schedule asks for annual amounts. Carry its annual total to page 1, as the schedule says.
Served, but no certificate
Page 3 ends with a certificate of service. Fill in who got a copy, where, and how, before you file. The form names who to serve: the people in the complaint, anyone who appeared later, and anyone the court names.
After you file
The county’s Guardianship Monitoring Program reviews the inventory. R. 4:86-1(c) (opens in a new tab) It checks it against the asset certification filed with the complaint.
