What you need
Have every account’s statements for the whole year and last year’s Packet C. In your first year, use the inventory instead.
The year starts on the date the court entered the order appointing you. The packet’s own example counts the 30 days from the year’s last day. The rule’s wording can be read as one day later, so file by the earlier date.
The example follows Lavina Brandt, an invented protected person. The order appointing her conservator was entered 03/12/2024. This is the second report, covering 03/12/2025 to 03/11/2026, and it is due 04/10/2026.
Each ending balance should match the Updated Inventory
The accounting has one page per account. It starts from last year’s ending balance, lists every amount received and paid, and ends on the balance the Updated Inventory shows.

The sale
Lavina held 100 fund shares. Her conservator sold 40 for $2,610.00 and deposited it in checking. The proceeds go on the checking page as an amount received. The packet has no line for a gain or a loss.
The 60 shares left go on the Updated Inventory at their present value on the year’s last day: $4,026.00.
Who gets a copy
Mail the finished packet to every interested person who has not waived notice, with the Notice of Right to Object. §6-1442(B) (opens in a new tab) The packet’s worksheet says that includes any government agency paying benefits and the bonding company.
Lavina’s Social Security is paid to her conservator, so the Social Security Administration gets a copy. Each person has 10 days from filing to object.
After you file
A clerk magistrate, court staff or the guardian ad litem reviews the packet. Problems go to a hearing. §6-1442(G) (opens in a new tab)

The court mails a reminder 45 days before each due date. If the packet is still missing 30 days after it is due, the court must order you to show cause why you should not be removed. §6-1442(F) (opens in a new tab)
The final accounting
When the conservatorship ends, file the Final Accounting Packet, CC 16:2.54 (opens in a new tab), with a motion to terminate. It covers the end of your last report through the day you file. No statute or rule sets a day count for it.

The final packet goes to every interested person, including those who waived notice of the annual reports. Neb. Ct. R. §6-1433(G) (opens in a new tab) The discharge order approves it and releases your bond.
Common mistakes
Mailing the bank statements
Statements go to the court only, with all but the last four digits of each account number blacked out. Interested persons get the packet without them.
Leaving out debit card payments
The packet says debit transactions must be included. List each one like a check.
Filing the checkbook register
Keep the register out of the packet. Use the accounting pages, or attach a software report or spreadsheet with the same columns.
Paying yourself without an order
Your fee and your attorney’s need a court order first. Apply for last year’s fees on CC 16:2.29 when you file the packet. Neb. Ct. R. §6-1443(C) (opens in a new tab)
