What it asks for
List all of the ward’s property that you hold or know about, with a value for each item. At the top, tick Conservator for Disabled Person.

Common mistakes
A loan left off Part 3
The statute asks for every encumbrance, lien and secured claim on each item. A car loan or a mortgage goes in Part 3 even when the car or house is listed above it. KRS 387.710(1) (opens in a new tab)
An empty Part 6
Money owed to the ward and any claim the ward could bring belong here: a personal loan, an insurance claim, a lawsuit.
No copy for the ward

Whole account numbers in Part 4b
Redact account numbers, Social Security numbers and birth dates before you file. CR 7.03
Property found later, never reported
File a supplemental inventory on the same form within 60 days of learning of it. The form’s notice (opens in a new tab); for a minor, KRS 387.100(1) (opens in a new tab)
The clerk reports missed inventories to the judge each month. A fiduciary who misses the extended date faces a show-cause hearing. It can end in removal or a fine of $100 a day. KRS 395.255 (opens in a new tab), KRS 395.990 (opens in a new tab)
