What it asks for
Parts A and B come first. They ask for contact details, whether the assets cover the person’s needs, the scope of your powers, and the bond. Then come seven steps.

Assets go in at their value as of the date of appointment, the date of the order appointing you.
The plan must rest on the person’s actual needs. The statute also asks for an estimate of how long the conservatorship will last, and for steps to help the person manage property again. §15-14-418(3) (opens in a new tab)

Common mistakes
Blank lines in the plan
Put “0” in both columns for a category that doesn’t apply. Don’t leave it empty.
Monthly and yearly figures that disagree
Multiply a monthly amount by 12 for the yearly column. Divide a yearly amount, such as property tax, by 12 for the monthly one.
Ticking “None” without checking
Each “None” box in Step 3 swears that the person owns nothing of that kind.
Assets found after filing
File an amended inventory when you find more property or a value proves wrong. File an amended plan when spending has to depart substantially from the approved one. §§15-14-418(5), -419(2) (opens in a new tab)
